The Mental Health Worker, can be named support, outreach, rehabilitation, or life skills workers, fulfill a generalist role on the team. They can have a variety of backgrounds from social services along with a passion in working in mental health and supporting ACT participants with their health and wellness care plan. 

The role of the Mental Health Worker includes, but is not limited to, these duties:

  • Care Coordinator provides service coordination when supporting daily needs of clients
  • Supports and Connecting clients with community programs and integration
  • Provides observational feedback to contribute to client assessment and monitoring
  • Supports client’s independence in activities of daily living and instrumental activities of daily living.
  • Promotes psychosocial rehabilitation services and groups, instilling hope and recovery
