The primary care can be provided by a Nurse Practitioner or a General Practitioner, who have experience and passion in providing collaborative, holistic health and wellness for people with severe and persistent mental illness. 

The role of the Primary Care Provider includes, but is not limited to, these duties:

  • Provides full service longitudinal primary care including screening, assessment, and management of acute and chronic health issues.
  • Collaborates with ACT psychiatrists and service providers to support medical care plan.
  • Provides wholesome health counselling to support health and wellness. 
  • Provides medication prescriptions and renewals.
  • Refers to diagnostic health services and other agencies as appropriate.
