Referral Information

Please note that incomplete referrals will not be processed and that the referral source may be invited to a teleconference or virtual meeting with the ACT Referral Screening Committee if additional information is required in order to make a more informed decision about whether to accept or decline.

Please also note that referrals cannot be made to a specific ACT Team. All referrals are reviewed by members from both ACT Teams. Team assignment and anticipated admission date is decided after a referral is accepted and is based on each team’s capacity for intake.

 

Your Kelowna ACT Teams:
 

  • Assessment: Both psychodiagnostic and cognitive testing (e.g., IQ, memory, learning, attention, executive functioning, academic) to inform case conceptualization and care planning. Assessment typically will involve interview with the client, collection of collateral information (e.g., interview with family members or informal supports, ACT Team members, other formal supports as indicated, chart review), and formal testing.
     
  •  Therapy: Empirically-based psychotherapy services for a variety of mental health concerns, including, but not limited to, mood, anxiety and related disorders, post-traumatic stress disorder, psychosis, and externalizing disorders. In particular, clients can be referred for CBT for psychosis (CBTp), cognitive remediation, and social skills training. 
     
  • Consultation: Client- consultee- and program-centered consultation. This also includes mental health education and in-service programming.

  • Our teams have 3 bikes and 2 chariots that we use to complete outreach to our clients
  • Our teams use fleet vehicles
  • Full time nursing support (blend of RN and RPNs), Substance use clinician (Masters level), SW, occupational therapist (OT), occupational wellness coach (vocational rehab), life skills workers, peer workers.  Leadership mix of SW and nursing.
